Patient Care in the CMSA Diabetes Program


Thank you for your interest in the CMSA Diabetes Program. At CMSA we use the team approach to diabetes treatment, which is used at most specialized diabetes clinics. The diabetes team is comprised of the patient, the CMSA Diabetes Team and importantly you, the patient’s own primary care doctor.

We are in the middle of a diabetes epidemic. We strive to help as many patients with diabetes as we can, and to do so, we ask you to help as much as possible. If a patient is doing well, we may ask him or her to see us just once a year, or even less often, while seeing you in the meantime. We will stop writing for medication refills if the patient had not been seen by us for two years, but that does not mean they have to return. If your patient is doing well, we would appreciate if you could take over prescription writing for their diabetes medications. However always feel free to contact us if things don’t seem to be going well. In addition, once a patient has been treated by our diabetes team, that patient can return for a follow up appointment usually within just a few days, even if he or she had not been seen in may years.

When you first refer a patient, we appreciate receiving a letter detailing what the issues are and what you are looking for, with all pertinent laboratory data attached. Also please keep us copied on relevant laboratory data that you obtain into the future. And don’t forget those insurance referrals when required.

Metabolic Diseases Associated with Diabetes

As you well know diabetes does not just start one day, but rather related metabolic abnormalities can be found even in children. In fact type 2 diabetes is part of a broader condition often referred to as metabolic syndrome, which is a risk factor for type 2 diabetes and vascular disease.
In addition about half the cases of hypertension are part of the metabolic syndrome. Metabolic syndrome is also now the leading cause of infertility in the US (through polycystic ovary syndrome; PSOS) and it is the leading cause of cirrhosis through Non-Alcoholic Steato-Hepatitis (NASH). Many cases of cancer are also being attributed to metabolic syndrome.

We will gladly see any patient with insulin resistance, metabolic syndrome, pre-diabetes, hyperlipidemia, overweight/obesity, PCOS, NASH, etc. Consider the diagnosis of metabolic syndrome in your patients with hypertension, CAD and stroke.
