We prefer that you send us your blood sugars by fax or e-mail for review, but there are other methods you may use if you do not have access to this technology or are uncomfortable with transmitting your health information over telephone lines. Please be aware that e-mail is not a secure means of communication.

Under most circumstances we will respond to you within 24 hours, however blood sugars we receive on Friday will be addressed on Monday. Since faxes, letters, phone messages, and even occasionally e-mails get lost, feel free to contact us again if you do not hear from us within 24 hours.

If you have an urgent need and are looking for a response in less than 24 hours, you must contact one of our nurses by phone at (814) 234-8800, and for emergencies please call 911 or go to the nearest Emergency Room. Please remember our diabetes nurse and nutritionist are involved in diabetes clinics most mornings and afternoons. They cannot always call back immediately while involved in patient care. Most phone calls to patients will be made before or after patients are seen.

Please do not send prescription requests via e-mail.

Blood Glucose Logs

Standard Log: MS Word, MS Excel
Gestational Log: MS Word, MS Excel
Pump Log: MS Word, MS Excel

Blood Glucose Log Instructions: You will need either MS Word or Excel to use these files. Right-click the file you want. From the pop up menu, choose "save target as ..." (Internet Explorer)" or save link as…" (Netscape). Pay careful attention to where you save the file, for example: C:\My Documents\GlucDiary2.xls. Open the file and enter your data. You can copy and paste the rows to increase the size of the table. Always save the file when you are done working. Please do not save your files in Office 2007 format. To e-mail your log to us, just attach the file to your e-mail and send to bsg@centremedsurg.com.
